When you work with me, you’re agreeing to the following TERMS & CONDITIONS:

  1. Client will clearly communicate goals and limitations to Trainer, and complete the required forms and assessments prior to beginning the training program. Client will inform Trainer if goals, health status, or limitations change in the course of the training program.

  2. Trainer will provide a consultation, assessment, and a series of planned workouts (Sessions) for the agreed upon time period.

  3. Client must arrive on time to each Session. Clients arriving late must inform Trainer as soon as possible, and will receive the remaining scheduled Session time.

  4. No shows for Sessions that are not rescheduled or cancelled 24 hours in advance will result in forfeiture of the Session. No refunds will be issued for any reason.

  5. Clients must sign the Session Tracking Form upon completion of a session.

  6. Payment is due at least 24 hours prior to Session to be scheduled by Trainer. Session dates/times are not guaranteed unless pre-paid. No refunds will be issued. If Client must reschedule Session for any reason, 24-hour advance notice is required.

In addition, you must have signed a Waiver of Liability prior to working with me.
